What is Age-Friendly care?
Age-Friendly care is health care that addresses your unique needs and wants. It can help you enjoy a better quality of life.
It is care that is safe and based on what research shows are the most important things to pay attention to as we get older — the 4Ms: what Matters, Medication, Mentation and Mobility.
Hospital discharge Oct 1 with leg wrapped ankle to hip for compression to stop bleeding from artery knicked during an outpatient procedure. MHN nurse came Fri Oct 3 to draw blood for labs to check hemoglobin and to unwrap/ rewrap hematoma area and measure swelling. She rewrapped and bandages fell off as soon as she left. She said next nurse coming Monday Oct 6.
NO nurse called til Oct 11 saying she would be there in 30min.I had notice and asked what she was coming to do based on Oct 3 nurse notes. She said there were no notes, she was coming to
assess needs.Fortunately, a dear friend whose also a nurse was coming every other day to do my dressings and tend to my care.I told MHN nurse to have her supervisor to call me.Its Oct 18 and no supervisor call.A 3rd nurse called Oct 15 and Oct 16, I declined their calls.
MHN PT came Oct 6 "to assess" and gave 3 exercises. The real therapist was to call later that day or Oct 7th.No one
called until Oct 15th.I declined the PT.
Grateful for friends.MHN left me on 2nd floor, no elevator, almost immobile and unable to navigate stairs.I'm 76.
