Honolulu is the capital and largest city of the U.S. state of Hawaii. It is an unincorporated part of and the county seat of the City and County of Honolulu on the island of O'ahu. The city is the main gateway to Hawai'i and a major portal into the United States. The city is also a major hub for international business, military defense, as well as famously being host to a diverse variety of east-west and Pacific culture, cuisine, and traditions. The Elderly Affairs Division (EAD), a division of the Department of Community Services of the City and County of Honolulu, is the sponsoring agency for Honolulu's Aging & Disability Resource Center (ADRC) program. Services Include:
Adult Day Care: Provides care outside the home. Participants have the opportunity to interact with others while being part of a safe and structured environment. Centers typically have staffed activities such as music and exercise programs and discussion groups.
Disease Prevention and Health Promotion: Health screenings, nutrition counseling and education and health maintenance programs to prevent and mitigate the effects of chronic diseases including osteoporosis, hypertension, diabetes and heart disease.
Information and Assistance: A service that (a) provides individuals with information on services available within communities; (b) links individuals to the services and opportunities that are available within the communities; (c) to the maximum extent practical, establishes adequate follow-up processes.
