Thank You from the Bottom of My Heart
Traveling with my elderly parents from Florida to California was a journey filled with both hope and concern—especially for my 92-year-old father, who was dealing with health issues, including severe weeping edema that required wound care. His physician recommended Transition Care Telemetry, Inc., and with one phone call, I knew we had found something special. There was something about Brian, the administrative contact, that felt so warm and endearing—it instantly put me at ease and confirmed we were in the right hands.
From that first moment, we were embraced with compassion and professionalism. I soon met Pam, who, to my surprise, was the CEO—and yet, she cared for my father as if he were her own. My dad is my world, and my goal was to keep him out of the hospital. With Pam's help, we accomplished that.
For seven months, Pam gave not only expert wound care, but also genuine companionship and kindness to my father. Her warmth, patience, and consistent presence brought so much peace to both him and me. As his condition progressed and hospice became necessary—a completely unfamiliar and overwhelming path—Pam remained by my side, guiding me with unwavering support and grace.
The team she has built is equally extraordinary. Katherine, the social worker, is another gentle soul whose guidance and comfort, provided calm through some of the most difficult moments.
As I write this, my dad likely has only a day or two left on this earth. Though my heart is heavy, I feel deeply grateful for the love and care he received. People come and go in your life, but some leave lasting marks. Pam, and the entire team at Transition Care Telemetry, will always have my deepest gratitude for helping me give my father dignity, comfort, and love in his final chapter.
Thank you—for everything.
With love and appreciation,
Pat Conklin
